For first applications and renewals, the doctor reviews health factors relevant to safe driving and completes the appropriate driving-licence medical form. This is separate from DLT eyesight, reaction, theory and practical tests. Bring your ID or passport, your existing licence if renewing, and relevant treatment records. Arrange the examination within the required 1-month submission window.

Medical Certificates in Bangkok
The right medical document starts with the right assessment. Explore Kauri’s five certificate services for driving, work, food handling, sick leave and general health declarations—with clear prices and preparation.
Which medical certificate do you need?
Match the document to the task you are completing. Each option links to a detailed guide with its examination, preparation and submission requirements.
Swipe or scroll across the table to compare prices and timing.
| Certificate | Choose it for | Clinic fee | Clinic turnaround |
|---|---|---|---|
| Driver's Licence Medical Certificate | A new or renewed Thai car or motorcycle licence | ฿200 | About 30 minutes |
| Work Permit Medical Certificate | A Thai work-permit application or renewal requiring medical evidence | ฿500 | 30–45 minutes; usually same day |
| Food Handler Medical Certificate | Preparing, cooking, serving or handling food | ฿1,290 | 3–5 days after sample receipt |
| Sick Leave Medical Certificate | Illness assessment and documentation for work or school | Doctor consultation: ฿500 | Timing confirmed after assessment |
| Five-Disease Medical Certificate | A general health declaration for employment, study or administration | ฿450 | Depends on the required assessment |
The sick-leave price is the doctor consultation fee. A sick-leave certificate can be provided after assessment when appropriate. Turnaround is the clinic’s preparation or result time, not the validity period of the document.
What does each certificate cover?
Similar names do not always mean the same examination. These are the practical differences between Kauri’s five services.
The relevant work-permit declaration addresses six specified conditions, including tertiary syphilis. Kauri’s pathway includes medical history, vital signs, physical examination, syphilis testing and drug screening. This is not the same as a general five-disease certificate. Some special migrant-worker programmes use designated facilities; use the detailed guide to understand the distinction.
Read the full guideFood-handler assessment focuses on health and infection risks relevant to food safety. Kauri’s listed pathway includes physician examination and stool testing. The sample kit and return instructions are provided at the visit; results take 3–5 days after sample receipt. Medical documentation and food-hygiene training are separate requirements, so keep both records.
Read the full guideA doctor consultation at Kauri costs 500 baht. The physician evaluates your illness or injury, explains care and can provide a sick-leave certificate when appropriate. Rest dates reflect your condition and work or study needs. For employees covered by the Labour Protection Act, an employer may request medical evidence for sick leave of 3 working days or more.

The traditional list and current official forms are not identical in every setting; civil-service recruitment can use different current criteria. The doctor completes the assessment appropriate to the purpose, rather than treating every application as the same checkup.

The examination answers the medical question behind the document. A driving certificate focuses on health relevant to safe driving; a sick note focuses on the current illness and recovery; occupational certificates address the specified health criteria.
- Health history
- Current symptoms, relevant conditions, previous treatment, medicines and allergies help the doctor understand what to examine.
- A focused physical examination
- Relevant vital signs and physical findings are assessed. The document’s purpose determines which health issues need particular attention.
- Tests when the pathway calls for them
- A basic driving or sick-leave assessment does not automatically require a broad blood panel. Work-permit and food-handler pathways use the relevant tests described in their guides.
- The physician’s conclusion
- The doctor interprets the findings, explains any next step and completes the appropriate declaration. If results are still pending, the final document follows once the necessary information is available.
What should a properly completed medical certificate contain?
The exact fields depend on the official format and purpose. Before you submit your document, check these essentials with the clinic.
- Your identity
- Name and identifying details that match your passport or Thai ID.
- The examination date
- The actual date you were assessed—not a substituted earlier date.
- The medical purpose and findings
- The relevant declaration, findings and any necessary test results.
- Advice about rest or restrictions
- For sick leave or fitness assessments, the doctor’s supported recommendations where relevant.
- The issuing physician
- The doctor’s name, professional identification and signature as required by the form.
- The clinic details
- The issuing clinic’s details and any required stamp or contact information.
A certificate records the assessment performed; it does not replace a driving licence, work permit, food-hygiene training certificate or an employer’s leave approval.
What should you bring—and do you need to fast?
Bring your identification and a clear reason for the certificate. You do not need an employer-created form to start a standard certificate consultation. If you already have special instructions, bring them so any additional requirements can be addressed.
- Your passport or Thai national ID, with the name to use on the certificate.
- Your certificate purpose and the date you plan to submit it.
- Your medication list, relevant medical records and previous results.
- If you have old X-ray films or reports, please bring them; they may help the doctor make an assessment.
Fasting is not normally needed for a basic certificate examination, a standard syphilis blood test or the listed stool-test pathway. If you book other blood tests at the same visit, follow the preparation instructions for those tests. Continue prescribed medicines unless your clinician advises otherwise.
From your clinic visit to the completed document
How long does it take—and how long is it valid?
Keep three questions separate: how long the clinic visit takes, when required results are ready, and how recent the certificate must be when you submit it. A certificate completed today may have a different submission deadline from another type issued on the same day.
| Driver's Licence Medical Certificate | Issued within 1 month before the DLT application, counted from the examination date. |
|---|---|
| Work Permit Medical Certificate | Follow the current application checklist and physician-stated period. It is separate from the work permit’s duration. |
| Food Handler Medical Certificate | Bangkok guidance calls for at least annual health checks and a medical certificate usable for 1 month from examination. The separate hygiene-training evidence has a 3-year period. |
| Sick Leave Medical Certificate | Applies to the consultation and leave dates recorded by the physician, not a generic 1-month or 1-year period. |
| Five-Disease Medical Certificate | The receiving employer, school or authority sets how recent the examination must be. |
Plan the medical visit around your submission date. A work permit’s duration is not automatically the validity of its supporting medical certificate, and food-hygiene training renewal is different from a food-handler health check.
Patient questions
Medical certificates in Thailand: common questions
Where can I get a medical certificate in Bangkok?
Kauri Health Clinic provides the five medical-certificate assessments on this page at 3063 Sukhumvit Road, at BTS Punnawithi in Phra Khanong. The clinic is open Tuesday–Saturday, 9:00–18:00. Book the certificate type you need or ask the team on LINE.
How much does a medical certificate cost at Kauri?
Driver’s licence assessments are 200 baht, work-permit assessments 500 baht, food-handler assessments 1,290 baht and five-disease assessments 450 baht. A doctor consultation for illness and appropriate sick-leave documentation costs 500 baht. Choose by purpose, because the examinations and tests differ.
Can I get a medical certificate on the same day?
Timing depends on the assessment and required results. Kauri’s driving-licence visit usually takes about 30 minutes; the work-permit pathway is usually 30–45 minutes with same-day completion when findings and results are ready. Food-handler results take 3–5 days after sample receipt. The team confirms timing for sick-leave and five-disease visits.
Do I need to go to a hospital, or can a clinic issue it?
A licensed clinic physician can issue appropriate medical documentation after assessment. Some application routes specifically require a government hospital or a designated facility, so those instructions take priority. This distinction matters particularly for certain recruitment and migrant-worker programmes.
Can foreign residents and visitors book a certificate assessment?
Yes. Bring your passport and explain the purpose of the document. The medical visit is separate from your visa, residence evidence and other application paperwork. The receiving authority determines which supporting documents your particular application needs.
Can I use one certificate for every purpose?
Not automatically. Driving, work-permit, food-handler and sick-leave documents answer different medical questions. A general health certificate or a normal blood-test report may not contain the declaration or evidence needed for another purpose. Choose the appropriate assessment rather than relying on the document’s title alone.
Can I download a form or submit a photograph of the certificate?
A blank form can help identify the required format, but it is not a medical certificate until completed through the appropriate physician assessment. Whether a scan, photograph or original is accepted depends on the receiving organisation. Check that before submission and keep your original document.
Can a doctor document an illness that started before today?
Tell the doctor when symptoms began and bring relevant records. The physician can record the history you report and distinguish it from today’s examination findings. The examination date must stay accurate; the certificate cannot falsely state that a previous consultation took place.
Do I need an employer’s form before booking?
Bring your identification and a clear reason for the certificate. You do not need an employer-created form to start a standard certificate consultation. If you already have special instructions, bring them so any additional requirements can be addressed.
