Gonorrhea: symptoms, testing and treatment

Learn about symptoms, urine testing, treatment and retesting.

What is gonorrhea?

Gonorrhea, also spelled gonorrhoea, is a bacterial STI caused by Neisseria gonorrhoeae. It can infect the genitals, rectum or throat and may cause discharge or painful urination—or no symptoms at all. It is treatable with appropriate antibiotics. Kauri offers urine panels that include gonorrhea and provides medication after assessment, with follow-up based on the infection site and response.

How is gonorrhea spread?

Gonorrhea spreads through vaginal, anal or oral sex and can pass to a baby during delivery. A partner does not need visible symptoms to transmit it. Throat infection can follow oral sex and often has no symptoms. Condoms and barriers reduce risk. Infection in one site does not tell you whether another site is infected, which is why the discussion about exposure sites matters before testing.

What are gonorrhea symptoms, and when do they start?

When symptoms occur, they often start within 1–14 days. They can include burning urination, white, yellow or green penile discharge, increased vaginal discharge or bleeding between periods. Rectal infection may cause pain, itching, discharge or bleeding. Many cervical and throat infections are silent. Severe pelvic or testicular pain, eye symptoms, or fever with painful joints needs urgent assessment rather than waiting for a routine test result.

Does gonorrhea go away on its own?

Do not use symptom improvement as evidence that gonorrhea has cleared. Untreated infection can persist without obvious symptoms, spread to partners and cause complications such as pelvic inflammatory disease or inflammation around the testicles. There is no reliable self-clearance deadline to plan around. Correct treatment addresses the infection, while follow-up distinguishes recovery, reinfection and possible treatment failure.

Is a urine test enough to check for gonorrhea?

Urine can assess urogenital infection, and both Kauri urine panels include Neisseria gonorrhoeae. A negative urine result does not rule out throat or rectal gonorrhea; those sites may need separate swabs. Molecular tests are commonly used for diagnosis. If symptoms persist after treatment, a clinician may request culture and antibiotic-susceptibility testing to investigate resistance. Tell the team about recent antibiotics and oral or anal exposure before choosing a test.

Which medication treats gonorrhea?

Ceftriaxone injection is a standard recommended treatment for uncomplicated gonorrhea. Additional treatment may be needed if chlamydia has not been excluded. Kauri provides medication for gonorrhea after physician assessment; the plan considers infection site, allergies, pregnancy and local clinical guidance. Resistance makes the choice of antibiotic important. Do not use leftover antibiotics or assume that a medicine used for a previous episode is the right choice now.

When can I have sex again, and do I need a repeat test?

Avoid sex for 7 days after treatment and until partners have been treated and symptoms have resolved. Retesting at about 3 months checks for reinfection. Throat gonorrhea requires a test of cure 7–14 days after treatment; the clinician chooses the timing and method. Routine cure testing is usually unnecessary for uncomplicated genital or rectal infection treated correctly. Persistent symptoms need review rather than simply taking another antibiotic.

Patient questions

Gonorrhea: common questions

How soon after sex should I arrange testing?

Contact the clinic promptly if you have symptoms or a partner has tested positive. A very early negative result may miss a new infection, so the clinician uses the exposure date and test method to plan testing or a repeat sample. The panel's 24-hour or 3–5-day result time describes laboratory processing, not how soon after sex infection can be excluded.

Are gonorrhea and chlamydia the same infection?

No. They are caused by different bacteria but can cause similar symptoms and occur together. Both are included in Kauri's 7-target and 18-target urine panels, and treatment is selected for the infection found.

Can I have gonorrhea with no discharge?

Yes. Infection can be symptom-free, particularly in the throat or cervix. A partner diagnosis or relevant exposure can be a reason to test even when you feel well.
